The same 2 x 2 inch print as the passport photo, but the online DS-160 upload is stricter: it must be square and between 600 x 600 and 1200 x 1200 pixels. Glasses have not been accepted since November 2016. The commonly-quoted 240 KB file cap could not be confirmed from a primary source, so the tool simply keeps the file comfortably small rather than quoting a number we cannot stand behind.
