A photo already used in a previous passport is rejected. No reflections, shadows or penumbra, and no thick or striking frames if you wear glasses.
Where the published rules run out
Brazil has two regimes. At a consulate abroad the photo is 5 x 5 cm, US style, which is what this page covers. Inside Brazil the Federal Police photograph you at the appointment, and a 5 x 7 cm photo is only needed for applicants under five.
